There are two options for completing the application, one is electronic, and one is with a typewriter, envelope, and stamp:

• ELECTRONIC: Download the PDF form by clicking above; open it up in Adobe, Canvas, Correl, or similar application; lock the original PDF data; create a new layer, and on this new layer type in your personal information; resave as a PDF, and e-mail to the Education Chair.
• TYPEWRITER: Download the PDF form by clicking above; print it out, use a typewriter, mail it to Education Chair.
• MAPSS will not accept hand-written applications.
